Back about a year or so ago, China started to tightened visa grants in Hong Kong. An applicant is supposed to get the visa from his/her country of origin or residency unless there is a reason. In case of HK, the HKID will show the residency.

However. if the applicant holds a permanent HK ID with Chinese nationality, he/she is supposed to enter China on Hui Xiang Zheng. It is analogous to that a US citizen must have only US citizenship on US soil. In fact, Chinese interpretation is more relaxed in that a Chinese citizen from a SAR entering China on a foreign passport will not lose his/her Chinese citizenship.

    Update:
Both my mom and I went back to the China Human Resources Center again.

My mom got denied (again)... with the same reason. She entered the country with her 'Chinese' citizenship using her HKID and cannot get a visa to China on her US passport without relinquishing her Chinese citizenship. (She is on HK soil as a Chinese citizen and they can't recognize you as both a Chinese citizen and a US citizen at the same time, which is what they would need to do if they were to issue you a China visa based on your US passport right now) We asked and they said that if she left HK and re-entered with her US passport as a US citizen without a HKID, then there would be no problem getting a tourist visa to china.

I, who DID enter HK with my US passport (and got an immigration stamp) because my HKID is expired, had no problems applying for the China VISA this time around.

The rules have definitely changed recently, because I have several cousins who were in our situation before a few years ago who had no issues getting a 'return home pass' before. We've asked several times at two different China Travel Agencies about the return home pass and could not get this without having visited China before.
