The Marriott Courtyard Hotel in Shek Mun has monthly fitness memberships, just 10 mins on MTR from Ma On Shan.
None of the government centres have the facilities requested by the OP. Posting LCSD urls to any semi-serious gym thread is a waste of time.
There's a Physical out in Tai Po. Warning- it's the crappiest gym chain in HK, but they should have a squat rack and barbells. Not close, but out in the right direction at least. Not sure if they do monthly memberships though.
Anytime Fitness out in San Po Kong if your hours are flexible.

How do you infer "Bench press, squat racks are musts" to fit the shitty Universal cable operated machines found in an LCSD facility?
Closest 'gym'* to me by far is LCSD. It's great for retired people with lots of spare time and a penchant for free air conditioning. Not so much for someone who presses and squats... even someone who is relatively weak (e.g. someone like me).
I've only ever seen squat racks in commercial gyms, personal training facilities and the powerlifting section of SCAA. You don't find barbells in an LCSD facility or hotel gyms, probably because of liability reasons.
* on reflection, it's fairer to call it a 'fitness facility' rather than a 'gym'
