    Really? I doubt records were destroyed, the CC list on any government project is a long list of email addresses, the simplest and more likely scenario is the records were never created. If no forms, then logically no inspections and sign off were carried out, so what has MTR been doing for the last few years sitting in the office waiting for forms?

    Normally, a monthly progress and management report is prepared, circulated to all parties, so everyone knew the records where missing.

    ...the Highways Department will soon inspect the quality of the construction in other MTR stations, beginning with To Kwa Wan and Diamond Hill.

    if we say 60% are missing nobody will notice 40%+60% = 100% haha

    more damning they knew about the missing records months ago and said nothing

    Perhaps they were just hoping the government would extend its lax approach of the lead in the water scandal to the railways

    more boll8cks, in fact it is the government that sets the rules ffs - so now the construction industry should just ignore the government's rules

    every government project has one or more clerk of works (COW) they inspection the works make records.................where was the clerk of works ??????

    Leighton 'too busy' for MTR paperwork, lawyer says
    Months after it was revealed that 60 percent of the crucial Request for Inspection and Survey Checks (RISC) forms could not be found, Leighton lawyer Paul Shieh said this was because the company's staff were too busy to complete them.

    Shieh (Leighton)conceded that steel reinforcement bars bought for the project were the wrong size and shape for the couplers they were to be screwed into.

    But he denied a subcontractor's claim at the inquiry that it had been told to just "screw the bars in as much as possible", saying the problematic connections made were down to the subcontractor's poor workmanship.
    does not make sense, they bought the wrong materials but installed them anyway!

    That is a terrible defense if you are the lawyer. Actually, for Leighton's case it might be their best defense.
