I've long thought workplace safety practices are quite lax. Even in offices with office workers precariously using ladders while overreaching for a heavy box. I'm frankly surprised how 'few' fatal and serious accidents occur (or are at least reported).

Unfortunately it has always been the case. Though some countries are a lot more dangerous than others in the workplace
