Can some one please help me understand the difference between the MTR and the KCR trains. Why are they designated differently?
Also what do you get "extra" if you purchase a "first class" ticket as compared to a standard fare?

there is no KCR now, only MTR
KCR = Kowloon Canton Rail (taken over by MTR on 2 Dec 2007)
MTR=Mass Transit Rail
there are no extras on firt class ticket except comfortable seats.

Less crowded in first, nicer seats and usually quieter. Trains can get REALLY crowded at times, but you're usually going to get a seat in first class for your extra money.

I think originally, the old 'KCR' was designed as a commuter train, and had more seating than now. The trains appear bigger, perhaps because most of the line were not in tunnels. The MTR is more like the London tube trains; they are designed for shorter journeys, so less seating, and more compact so that they can fit in a tunnel. I'd suppose that a wider train would mean a bigger tunnel meaning more expense.
tsuen wan line from central to tst it gets really crowded, sometimes you're literally in crushed can of sardines, even worse if some has bo next to you.
former kcr east line from sheung shui to hung hom, during normal hours you won't get a seat unless you board form sheung shui or hung hom and rush in to the seats, that's why there is first class department
