If you have a HKSAR passport it should be sufficient. You go apply at the China Travel Agency:

I've just applied...they asked of me:
1) HK passport (2 copy & original needs to be sighted)
2) HK ID (2 copy & original needs to be sighted)
3) other passport* (2 copy & original needs to be sighted)
* Depending on it's says on your HK ID about your first date of issue (mine was only last year) they might ask you for other passport (to view details of initial entry to HK)
4) passport photo (x 2)
5) Completed application form (this only comes in chinese, and must be completed likewise)
You have to queue up in the morning at China Travel Service before they open, and get a ticket (they have limited quota per day) and lines can be very long. They open at 8am and I got there at 7am with already quite a number of people in queue.
They told me I should get it within 4 weeks.

I submitted only my HKSAR passport and two copies of my HKID. They didn't ask for my U.S. passport or original HKID.
I didn't get my HKID until I was in my twenties, because I grew up in the U.S., and I didn't switch to a smart card till about a decade later, because I didn't get around to it on visits to HK. And I just got around to getting my HKSAR passport. I picked up my HKSAR and went straight to CTS to apply for a permit.Â
The CTS rep who took my application said they might hold up my application because I waited so long to get all my documents, but they didn't. I paid 200 HKD extra for a rushed permit and got it four business days from date of application.
I didn't make an appointment. I just showed up at late afternoon mid-week, and took a number. I was out of there in about 25 minutes, tops. I went to the CTS office in Kun Tong (spelling?). A relative told me the office was moved there recently, so should have relatively short waits.

No idea. I can only speak about my experience.
My relatives said I probably lucked out because many HKers are eager beavers who like to get stuff done early. I didn't know whether I'd be able to leave work early to pick up my HKSAR and then head to CTS. Turns out I was able to leave work at 3 p.m. and made both with plenty of time to spare.
My relatives said that if the CTS office serves its quota of people for the day, no matter the time, they stop taking apps. I dunno whether that's true. So I guess I could've gotten shut out, but I didn't. It helps to not go when there's a rush, like during summer, I was told. Again, I can't speak beyond my experience.
