Documents for dependent visa

Hi, what documents we need to provide to HK immigration dept to apply for dependent visa for my husband and baby??

Both of them were born in Belgium, Belgian nationality. I have Belgian nationality and permanent HK ID card and passport.

My husband could not find his own birth certificate, is it necessary for dependent visa??

I have searched the immigration dept website but could not find exactly where it is, can anyone help?
