I don't know your purpose of getting this, but if it is for leisure I would personally just get a multiple entry visa, which is much easier and faster. I believe you will need a HKPID to build a village house and be registered in your village administration office to do so.
    are you living in Hk now? have you successfully applied for a HK SAR Passport? if you want to go to the mainland, obtaining a visa is not hard, though it can be pricey depending on what passport you hold.

    Imp. How do you know what you wrote ?
    12 months ago ID n passport n declaration ( which stated the timeline of when you obtained these docs) was suffice
