Edit: I suppose, depending on your imagination, it could be quite a few things! My initial thoughts were different to the one's after I came back to have another look.

Two ones from UK Universities:
Newcaslte Poly renaming as a University - first choice which got a long way through the system before the problem was spotted was: City University of Newcastle upon Tyne - abbreviated to?
Humberside Uni who, at great expense, designed and displayed a large sign showing a male student passing a baton to a female student, male student facing female student. Unfortunately it was in silhouette and the 'baton' was at hip height. The logo lasted a few weeks before it was withdrawn.
